INR Clinic

The INR Clinic is run by pharmacists and nurses within the Guelph Family Health Team. By taking a drop of blood from a patient’s finger and applying it to our “Coagucheck” machine we are able to get your blood level very quickly. There is no longer a need to go to the lab to have blood taken from your arm. Before you leave the clinic you will have your result, you will know your dose of warfarin to continue with, and you will know when to return to the clinic for your next visit. Foot Care Clinic

Foot problems are common in people with Diabetes and other chronic conditions and can lead to serious complications. The GFHT Foot Care Clinic aims to provide patient education and individualised foot care services to prevent foot problems and potential serious complications, teach foot care self-management including reducing foot injury, proper foot wear or self inspection and recommend follow up with appropriate healthcare professional where indicated. Get FHT Program

The Guelph Family Health Team is providing a program to encourage healthy lifestyle habits in patients with risk factors for Heart Disease and Diabetes. Get FHT is based on a collaborative approach between the Patient and the Family Health Team and will take the format of 3 group educational seminars followed by one-on-one counselling and support. Patients can expect to be active in the program for 6 months followed by the opportunity for additional education and support sessions. Diabetes Care Guelph

Diabetes Care Guelph (DCG) is available to all residents of Guelph.
DiabetesCare Guelph is funded by the Ontario Ministry of Health and Long-TermCare in support of the Ontario Government’s decision to make diabetesprevention and control a major health priority in our community.Operated as a lifestyle program of the Guelph Family Health Team forall residents of Guelph, it offers a wide range of programs andservices designed to help you reduce your risk of complications fromdiabetes and help you manage the disease.

Our mission at Diabetes Care Guelph is to provide knowledge,resources and health management programs to assist all people withdiabetes and their families in developing the skills required foreffective self-management for their diabetes.

Aging At Home

The Guelph FHT Aging at Home program provides in-home nursing visits to elderly patients of the Guelph Family Health Team.  The goal is to reach seniors who are at risk for social isolation, unsafe living conditions, or lack of access to resources, before these issues become crises. Through Aging at Home support, family doctors have a better understanding of their patients’ needs and patients are able to stay in their chosen homes longer and may avoid unnecessary Emergency Visits or hospitalization.

Spirometry Clinic

This clinic offers pulmonary function tests (breathing tests) for patients who have lung health issues. It is also used to help diagnose and manage illnesses such as asthma and chronic obstructive pulmonary disease (COPD). Your Doctor or Nurse Practitioner will discuss this test with you and determine if it is a test you may need. If you have any concerns regarding your lung health, please contact your healthcare provider.
