- Two times as likely to die from that heart attack or stroke
- At a five time risk of developing Type 2 Diabetes
How can Get FHT help me manage Metabolic Syndrome?
Our Get FHT program is open to patients of Guelph Family Health Team practitioners.
The goal of this program is to prevent or delay the progression of heart disease or diabetes.
It requires the time commitment of one year; the first month requires daytime appointments that are 1.5 to 2 hours in length, for individual appointments with a Nurse, Dietitian and exercise specialist.
After this, follow ups are for 30 mins every 4-6 weeks.
