Group Services:
After we receive the referral from your physician you will be contacted to attend a “Introduction to Diabetes” class. If you have not been referred to the program, you are still welcome to attend classes. Please contact the Diabetes Care Guelph office at (519) 840-1964 for more information.
Self-Care Management:
Evidence has shown that teaching Canadians how to be partners in their own health helps them stay healthier. Programs encouraging self-management for diabetics have produced important improvements in blood sugar and blood pressure levels, which in turn lowers complications of heart attacks and strokes.
To help you to manage your diabetes, individual appointments are available. You can call to request a follow-up appointment with the various health care providers on the team.
