Mission

Diabetes Care Guelph (DCG) is funded by the Ontario Ministry of Health and Long-Term Care in support of the Ontario Government’s decision to make diabetes prevention and control a major health priority in our community. Operated as a lifestyle program of the Guelph Family Health Team for all residents of Guelph, it offers a wide range of programs and services designed to help you reduce your risk of complications from diabetes and help you manage the disease.

Diabetes Care Guelph works closely with the Guelph Community Health Centre, Guelph General Hospital, local physicians and other health services in our community to ensure people with diabetes have access to the information and resources they need to manage their diabetes.  

Our mission at Diabetes Care Guelph is to provide knowledge, resources and health management programs to assist all people with diabetes and their families in developing the skills required for effective self-management for their diabetes.
