Diabetes Care Guelph:

If you would like to refer to Diabetes Care Guelph, please contact us at 519 840 1964.  We would be happy to further discuss and send you a copy of our referral form if applicable.

We will require recent lab work when referring a patient, including: HbA1c, FBS, Lipid profile and kidney function tests.

If you are a practitioner with the Guelph Family Health Team and would like to refer a patient, please contact us at 519 840 1962.
