Program Outline

Diabetes Care Guelph:

Open to all patients who have been diagnosed with diabetes AND either live in Guelph or have a physician in Guelph.

Target patients are anyone who has diabetes.

• After we receive the referral, the patient will be contacted to attend an intake appointment. This joint appointment with a nurse and dietitian will take about an hour. Health history is taken and care plan is discussed.
• Next step is typically a group seminar. This is meant to be an interactive discussion about diabetes and diabetes management.
• Follow up appointments are then booked on a patient specific basis.
• Additional Wellness series classes are also available.

Get FHT:

Open to patients of Guelph Family Health Team practitioners.

Target patients are people at risk for developing heart disease or diabetes.

• During the first month, the program will require weekly daytime appointments that are 1.5 to 2 hours in length.
• This includes one on one appointment with nurse, dietitian and exercise specialist. For the rest of the program, there will be 30 minutes of follow up appointments every 4-6 weeks.
• The length of the program is 1 year.
